Population health management (PHM) focuses on organizing and managing the health care delivery system in a way that makes it more clinically effective, more cost effective and safer. PHM encompasses the spectrum of care from doctors’ offices to emergency departments and hospital admission to continued care after discharge. By integrating care and providing better access to treatment, PHM helps to achieve the “Triple Aim” of improving health, enhancing the patient experience and lowering avoidable costs.
